HERE ARE SOME FORMS TO USE OR TO HELP DEVELOP YOUR OWN COMPANIES POLICIES AND HOUSE RULES:

Each type of license has its own specific rules and regulations. The licensee can use theses forms as they are or modify them for their specific needs.

There are three forms for each type of license:

1. There is a HOUSE GUIDELINE (RULES), this form should be posted for all employees to see, read and understand

2. There is an EMPLOYEE GUIDELINE form that the employer can make copies of for each employee to read and understand and sign. The signed copy should be kept with the employee records.

3. There is a POSTER GUIDELINE form that should be posted for all employees to see, read and understand. This is almost identical to the employee guideline.
